Pathway and Transition Planning Website
One of the initial concerns was the availability of up-to-date information, and a place where people could go to begin navigating the system. Thanks to the Department of Education, a designated portal has been created to assist young people with disability (and those who are supporting them) in their transition to adult life. The website includes guidelines, pathways, processes, resources and templates for young people, parents, carers and schools to prepare and support young people to move through school to adult life.

Transition Planning Resource
What is an Individual Transition Plan?
The Individual Transition Planning process prepares and supports young people to move as seamlessly as possible through secondary school to post school life. It is a process that helps young people plan, prepare and work towards achieving their future directions and goals. The Individual Transition Plan empowers young people, ensures effective support is in place, maximises choices, informs decision making and provides direction and purpose. To receive an electronic copy of the Transition package please fill out the form at the base of this page or by phoning (08) 9286 6600.
